Mid-AM Penalty Reporting Tool

 

The Mid-Am district has instituted a new mandatory on-line reporting procedure for all game misconduct, match penalties, fan ejections and rink issues.  We are now required to report these situations within 24 hours of the game using the Penalty Reporting tool found on the www.midamref.org web site.  http://www.offserv.com/midam/members/
  • We need to make sure that all coaches names (first and last) and phone numbers are on the score sheet before the start of the game.
  • Since the system times out after 20 minutes, it is suggested that you draft your description of the incident(s) using a word processor and then copy and paste it into the system.
  • You need to log in:
  • Your username is your last name
  • Your password is your USA Hockey ID number.
  • Only members paid for the current season and registered with USA Hockey may access this area.
  • You are strongly advised to change your username and password.
  • Once your report is submitted, you and your partner will receive a confirmation e-mail.  
  • You are requested to forward this confirmation to the WOHRA Board at board@wohra.org.
